# Annex ZA-2.4.1: Medical Record Tampering – System Access Screenshots
Between October and December 2022, Beizi Li’s psychiatric records were accessed and modified more than **129 times** without medical justification. These unauthorized entries involved the deletion, alteration, and falsification of diagnostic data.
This annex provides direct visual evidence of:
– Repeated access by multiple staff within short timeframes
– Retroactive editing of diagnostic labels and medication records
– Metadata patterns indicating deliberate manipulation
