Medical Glasses Withheld, Reported Missing, and No Parental Notification
This page documents how my child’s existing medical glasses, after being used for approximately one year, were later discontinued, reported as “missing,” and handled without notifying the legal guardian. No medical assessment, written justification, or replacement basis was provided during this process. The incident raises concerns regarding lack of medical decision-making basis and disregard for the guardian’s right to be informed.
• My child had previously been provided with medical glasses for daily use.
• The glasses were used for approximately one year, although the exact timeframe may be verified further.
• During institutional care, I was informed that the glasses had been “lost,” and they were no longer being used.
• During this period, the legal guardian received no notification regarding the loss of the glasses or how the matter had been handled.
• The glasses were later found again and returned, which created inconsistencies in the explanation provided.
• In the later period, the glasses again appear not to have been used or to have been discontinued.
• In other words, the same medical device was repeatedly subjected to “missing / found again / discontinued again” within a relatively short period.
• Throughout this process, no medical assessment or written basis was provided to explain these changes.
• The medical glasses were discontinued without any medical assessment being provided.
• The same device was repeatedly described as “missing / found again / discontinued again” within a short period.
• During these changes, the legal guardian was neither informed nor involved.
• No written records were provided explaining the basis for these decisions.
• No doctor’s assessment or medical note was provided.
• No vision re-examination or vision test record was provided.
• No medical reason for discontinuing the glasses was explained.
• No replacement or re-prescription recommendation was provided.
• The legal guardian was not notified of these developments.

Discontinuing their use without any medical assessment, while also failing to notify the legal guardian and allowing repeated “missing / discontinued” episodes, is not consistent with a handling approach genuinely based on the child’s best interests.
This incident shows that, in the absence of medical assessment and written justification, the relevant institutions discontinued the use of an existing medical device for a child, failed to notify the legal guardian, and provided inconsistent explanations regarding the device’s status. This raises concerns about both medical decision-making procedure and the guardian’s right to be informed.
Evidence on this page will continue to be supplemented.
The situation documented on this page shows that the use and management of an existing medical device for a child lacked clear medical basis and documentation, while the basic duty to notify the legal guardian was not fulfilled. This reflects procedural and decision-making incompleteness.
